Patient Forms

To decrease wait time in our office, we recommend you download and print our new patient forms. When you come to your first appointment, bring in these documents and make sure all the necessary information has been filled out and signed. This will make your appointment session much quicker and you won’t feel rushed during the on-boarding process.

Our office will gladly answer any questions you may have on our payment options, as well as estimate your insurance deductible. Should you wish to seek more extensive dental treatments, Sterling Dental Center also offers financing through CareCredit®.

Don’t have dental insurance? No problem! Taking care of your oral health should never be inaccessible, which is why we offer a range of payment methods. We are one of the few dental practices to offer in-office membership programs to simplify payments for patients without insurance. If you are insured, we accept most dental plans but recommend you call to double-check your coverage.

In order to assist you with your health care investment, we are providing the following payment options:
